Photo Shoot Questionnaire

Congratulations on being accepted as one of our summer camp photo shoot models. We are super excited to work with you! The photo shoot is coming up soon and it's going to be a lot of fun.

Please answer a few questions below and have a parent sign the Photo Release form. We need your response to these questions by this coming Monday, August 15th. After this you will receive one more email from us on Tuesday, August 16th. That email will contain instructions on what kinds of clothes we would like you to bring (you won't need to shop for new clothing) as well as information regarding the time for the photo shoot and directions to Kamp Kenwood.
